Implementation must take place in Alabama, Georgia, Kentucky, Mississippi, North Carolina, South Carolina, Tennessee, and/or W. Virginia. The Foundation will award up to **three, two-year grants of up to $750,000 USD each** to support the design, implementation and evaluation of survivorship care models focused on improving health outcomes, including physical and mental health, as well as overall quality of life (QoL) for lung cancer survivors, their family members and/or caregivers through: - Addressing the barriers to provision of quality survivorship care to disparate and vulnerable populations in your target area - Filling the existing research gaps by designing and implementing innovative interventions that improve access to quality survivorship care and optimizes health beyond cancer diagnosis and treatment - Collaborating with community-based service delivery providers such as PCP’s, oncologists, pulmonologists, radiologists, advocacy groups, payer groups, etc. to provide patient centric survivorship interventions to benefit hard to reach populations - Empowering survivors through education and community-based support services including self-management and surveillance of their health care needs The closing date to submit proposals is **August 26, 2016.** Please [click here](http://www.bms.com/foundation/cancer/Pages/cancer-us.aspx) to access the RFP and for more details on selection criteria, eligibility, and submission information.
